Has Superman ever had to act and save the day as Clark Kent? What were the repercussions, if there were any?

This seems extraordinarily broad. "Superman" covers nearly a century of films, TV shows, radio shows and more than 10,000 comic serials.

What would be a good improvement to narrow down the sheer size of possible answers. Suppose i could ask events with most significant outcomes?

The question is also really broad. Do you just mean not putting on his costume?

More like "oh gee how can good old Clark lift a 3 ton bus" moments eventually leading to people figuring out he and superman is the same person

The TV series "Smallville" is pretty much exactly this in every single episode. It's set before Clark becomes Superman so every time he uses his powers, he does it as Clark Kent. Also, virtually every movie includes at least one scene where Clark uses his powers to do something heroic. Usually, he does so in a way that won't compromise his identity so there are no consequences. Perhaps a better question is to ask for instances where Clark using his abilities has compromised his identity as Superman.
